changes, to differentiate ECT responders (n¼176) from non-responders (n¼64).Results: Our model achieved a maximum discrimination accuracy of 62.31%, significantly above chance-level performance (Z ¼ -32.20, p ¼ 0.0013), using two pairs of spatial filters.Spatial mapping revealed then significant rsFC patterns involving core regions of the default mode network (bilateral precuneus cortex, temporal cortex, medial preforantal coretx, inferior parietal lobule, left frontal pole and left ventral prefrontal cortex) and the cognitive executive network ( bilateral dorsal prefrontal cortex). An anticorrelation between these core regions and the bilateral insula part of the Salience Network (SN), right auditory cortex (superior temporal cortex), bilateral visual cortex (extrastriate cortex) contributed notably to the discrimination between responders and non-responders.Conclusion: Employing a new machine learning technique, this study, leveraging the largest ECT-fMRI dataset available, demonstrates key discriminative regions that differentiate ECT responders and non-responders at baseline. These regions include core regions of the DMN and CEN, with a critical anticorrelation observed with the bilateral insula, right auditory and bilateral visual cortex Research Category and Technology and Methods Translational Research: 2.
