Commentary on: Think Smooth and Pink: The Role of Skin Color and Texture in Caucasian Female Genital Aesthetics

Jankowski et al's study1 investigating the determinants of perceived female genital attractiveness provides a novel contribution to the field of aesthetic surgery. Historically, the focus of female genital cosmetic surgery (FGCS) research and practice has been overwhelmingly on labiaplasty, particularly the reduction of the labia minora.2 This emphasis has seemingly often been justified by cultural ideals, media representations, and assumptions about patient dissatisfaction, rather than empirical evidence regarding the factors influencing aesthetic perception. Jankowski et al1 challenge this perspective by evaluating relative contributions of vulvar skin perfusion, texture, and morphological features to perceived attractiveness using an innovative study design. Jankowski et al1 analyzed a large image database comprising 1080 vulvar images, ultimately manipulating 360 representative images to vary skin redness while keeping morphology constant. This allowed a controlled assessment of chromatic effects on attractiveness, a factor less considered in FGCS research. Attractiveness judgments were obtained from 159 adult participants (45 men, 114 women) through over 40,000 paired comparisons, analyzed using Bradley–Terry models and hierarchical regression. Notably, the study found that enhanced skin perfusion (redness) and smoother skin texture were strong positive predictors of attractiveness, whereas visible labia minora, skin wrinkling or atrophy, presence of a supralabial crease, paler or yellowish skin, and increased model age were negative predictors. These findings collectively explained 59% of the variance in attractiveness ratings, demonstrating that chromatic and textural characteristics of genital skin are highly important, alongside traditional morphological measures like labia minora size.
